The liver receives exclusively autonomic (involuntary) innervation via the hepatic plexus, which is a continuation of the celiac (solar) plexus.

  • Pain from liver distension: Acute stretching of the liver capsule (Glisson's capsule) causes right upper quadrant pain, which may be referred to the right shoulder via phrenic nerve innervation of the diaphragmatic peritoneum (C3–C5).
  • Gallbladder and bile ducts: Unlike the hepatic arteries (sympathetic only), the gallbladder and extrahepatic bile ducts receive both sympathetic and parasympathetic innervation.
  • The hepatic plexus accompanies branches of the portal triad into the liver parenchyma, influencing vascular tone, bile flow, and metabolic activity.
