Administration

  • Reconstitute 4 g vial with 37 mL NS 0.9% → gives 100 mg/mL
  • Infuse IV over 30 minutes (standard); extended/continuous infusion preferred for severe infections
  • Continuous infusion (Hospital-in-the-Home): 300-400 mg/kg/day; loading dose = 25% of daily dose over 30 min

Key Points

  • Dose is weight-based until child reaches 40-50 kg, then switch to adult dosing
  • For severe/resistant infections, q6h dosing or extended infusion improves time-above-MIC (PD target: fT>MIC)
  • Always combine with vancomycin for necrotizing fasciitis (mixed infections)
  • For febrile neutropenia, combine with an aminoglycoside
