Medical Surgical Nursing Kya Hai?

Sochlo ek hospital hai. Usme do tarah ke patients aate hain:
  • Medical patients - Jinhe dawai se theek kiya jaata hai (jaise BP, Diabetes, Pneumonia)
  • Surgical patients - Jinhe operation karna padta hai (jaise Appendix, Fracture)
Tumhara kaam hai - dono tarah ke patients ki care karna. Yahi hai Medical Surgical Nursing.

NURSING PROCESS - ADPIE

Yeh 5 steps hain jo ek nurse har patient ke liye follow karti hai.
Copy mein bada heading likho: "NURSING PROCESS (ADPIE)"

Ek Simple Example Se Samjho:

Socho tumhare paas ek patient aaya jise bukhar aur khansi hai. Ab tum nurse ho - kya karogi?

Step 1 - A = ASSESSMENT (Jaanch Karna)

Matlab: Sabse pehle patient ke baare mein saari information collect karo.
Tum poochhogi:
  • "Kab se bukhar hai?"
  • "Kitna temperature hai?"
  • "Khansi ke saath kuch aata hai?"
  • Haath lagakar check karogi - pulse, BP, temperature
Simple definition likho:
Assessment = Patient ki poori jaanch karna - physically aur poochhkar

Step 2 - D = DIAGNOSIS (Nursing) (Problem Dhundhna)

Matlab: Jo information mili usse samjho ki patient ko kya problem hai - nursing point of view se.
Jaise is patient mein:
  • Bukhar hai - toh problem = Hyperthermia (body temperature zyada hai)
  • Khansi hai - toh problem = Ineffective Airway Clearance (airway theek nahi hai)
Simple definition likho:
Nursing Diagnosis = Patient ki nursing problems identify karna
⚠️ Yeh doctor ka diagnosis nahi hota (jaise Pneumonia). Yeh nurse ka diagnosis hota hai (jaise breathing problem).

Step 3 - P = PLANNING (Plan Banana)

Matlab: Ab socho ki kya karna hai patient ke liye.
Jaise:
  • "3 din mein patient ka temperature normal ho jayega"
  • "Patient aaram se saans le payega"
Simple definition likho:
Planning = Patient ke liye goals aur targets set karna

Step 4 - I = IMPLEMENTATION (Kaam Karna)

Matlab: Jo plan banaya, ab actually kar ke dikhao.
Jaise:
  • Paracetamol dena
  • Nebu (nebulization) dena
  • Forehead pe thanda pani ki patti rakhna
  • Patient ko rest karwana
  • Poora paani pilana
Simple definition likho:
Implementation = Plan ke according actual nursing care dena

Step 5 - E = EVALUATION (Result Dekhna)

Matlab: Kuch din baad check karo - kya patient theek hua?
  • Bukhar gaya? ✅ Goal achieve hua
  • Bukhar abhi bhi hai? ❌ Plan change karo
Simple definition likho:
Evaluation = Dekho ki patient better hua ya nahi, agar nahi toh plan badlo

Vital Signs Kya Hote Hain?

"Vital" ka matlab hota hai "Zindagi se related"
Vital Signs woh 4 cheezein hain jo batati hain ki patient zinda hai aur uska body theek kaam kar raha hai ya nahi.

Copy mein likho:
Vital Signs = Woh measurements jo patient ki body ki condition batate hain

Vital Signs Kitne Hote Hain?

1. T  = Temperature     (Bukhar)
2. P  = Pulse           (Dil ki dhadkan)
3. R  = Respiration     (Saans lena)
4. BP = Blood Pressure  (Khoon ka dabaav)
Trick to remember: "T P R BP" - bas yahi yaad karo!

1️⃣ TEMPERATURE (T) - Bukhar

Kya hota hai?

Body ke andar kitni garmi hai - yeh measure karta hai.

Normal Value:

Normal Temperature = 98.6°F  (ya 37°C)
Copy mein likho yeh table:
98.6°F  = Normal
100°F+  = Fever (Bukhar)
95°F-   = Hypothermia (Body thandi pad gayi)

Kahan se measure karte hain?

2️⃣ PULSE (P) - Naadi / Dhadkan

Kya hota hai?

Dil jab dhakta hai toh khoon arteries mein push hota hai - uski speed aur rhythm feel karna.

Normal Value:

Normal Pulse = 60 - 100 beats per minute (BPM)
Copy mein likho:
60-100 BPM  = Normal
100+ BPM    = Tachycardia (pulse tez hai)
60- BPM     = Bradycardia (pulse dheemi hai)

Kahan se check karte hain?

Kaise check karte hain?

  • 2 ungliyaan rakhte hain (index + middle finger)
  • Thumb nahi lagate (kyunki thumb ki apni pulse hoti hai)
  • 1 minute count karte hain

3️⃣ RESPIRATION (R) - Saans

Kya hota hai?

Patient ek minute mein kitni baar saans leta hai - yeh count karna.

Normal Value:

Normal Respiration = 16 - 20 breaths per minute
Copy mein likho:
16-20  = Normal
20+    = Tachypnea (saans tez chal raha hai)
16-    = Bradypnea (saans dheema chal raha hai)
0      = Apnea (saans band ho gayi - EMERGENCY!)

Kaise check karte hain?

  • Chest ka upar neeche hona dekho
  • Patient ko batao nahi - warna woh apni saans change kar leta hai
  • Pulse check karte karte hi secretly count karo

4️⃣ BLOOD PRESSURE (BP) - Khoon ka Dabaav

Kya hota hai?

Khoon jab arteries mein baahta hai toh kitna pressure dalta hai - woh measure karna.

Do Numbers Hote Hain:

Systolic  = Jab dil dhakta hai (upar wala number)
Diastolic = Jab dil aaram karta hai (neeche wala number)
Jaise: 120/80 mmHg
  • 120 = Systolic
  • 80 = Diastolic

Normal Value:

Normal BP = 120/80 mmHg
Copy mein yeh table banao:
120/80      = Normal ✅
140/90+     = Hypertension (BP high - DANGER)
90/60-      = Hypotension (BP low - DANGER)

BP lene ka saamaan = Sphygmomanometer (BP apparatus)
