Urine examination changes are most dramatic and diagnostically central in the renal group (nephrotic syndrome shows heavy proteinuria with fatty casts; nephritic syndrome and lupus nephritis show hematuria with RBC casts; AKI/CKD show variable proteinuria, casts, and specific gravity changes; UTI shows pyuria, bacteriuria, nitrites). Diabetes mellitus (group 6) is the other major contributor, with glycosuria and ketonuria. Hemolytic conditions (severe malaria, sickle cell crisis, autoimmune hemolysis) cause hemoglobinuria, while liver disease (hepatitis, cirrhosis, jaundice) alters bilirubin and urobilinogen in urine. Muscle-damaging conditions (rheumatic disease flares, muscular dystrophy, severe cellulitis with rhabdomyolysis) can cause myoglobinuria. Most conditions in the GI, respiratory, dermatology (non-endocrine), neurology, and musculoskeletal-mechanical groups (like OA, frozen shoulder, cervical spondylosis) have little to no specific urinary finding beyond nonspecific effects of dehydration or acute illness.
