Elderly male dm, amputated toe, htn old ischemic stroke right hemeparesis. Used to walk with walker. Complaints of difficulty in walking, lifting both limbs, and increased weakness of right upper limb. Also c/o back ache. Examination vitals stable. Hmf slightly confused, slow replying. Speech normal. Cranial nerve normal. Tone increased in right lower limb and right upper limb. Reflexes absent in lower limb. Plantar b/l mute. Ct report attached. Labs tc 15k,na 130 rest all are ok
