Koch's Postulates

Koch's postulates (also called "Kocha's postulates") are four criteria formulated by Robert Koch and Friedrich Loeffler in 1884 (refined by Koch in 1890) to establish that a specific microorganism is the cause of a specific disease. They remain a foundational concept in medical microbiology and pathology.

The Four Postulates

  1. The microorganism must be found in abundance in every case of the disease, and should be absent from healthy hosts.
  2. The microorganism must be isolated from the diseased host and grown in pure culture (outside the body, in a lab medium).
  3. The cultured microorganism should cause the same disease when introduced into a healthy, susceptible host (e.g., an experimental animal).
  4. The same microorganism must be re-isolated from the newly infected experimental host and shown to be identical to the original causative agent.
As summarized in Bailey and Love's Short Practice of Surgery: "Koch's postulates proving whether a given organism is the cause of a given disease - it must be found in every case; it should be possible to isolate it from the host and grow it in culture; it should reproduce the disease when injected into another healthy host; it should be recovered from an experimentally infected host" (Bailey and Love's Short Practice of Surgery, 28th ed., p. 72).

Classic Examples

1. Tuberculosis (Mycobacterium tuberculosis) This is the disease Koch used to originally establish and prove his postulates in 1882.
  • M. tuberculosis was found in the lung tissue and sputum of every TB patient examined, and not in healthy people.
  • It was isolated and grown in pure culture on a special solid medium.
  • Guinea pigs inoculated with the pure culture developed tuberculosis.
  • The organism was re-isolated from the infected guinea pigs and matched the original strain.
2. Anthrax (Bacillus anthracis) Koch's earlier (1876) work on anthrax was actually the first full demonstration of the concept.
  • Rod-shaped bacteria were consistently found in the blood of animals that died of anthrax.
  • The bacteria were cultured in a nutrient medium outside the animal body, through multiple generations.
  • When injected into healthy mice, the cultured bacteria produced anthrax.
  • The same organism was recovered from the newly infected mice.
3. Cholera (Vibrio cholerae) Koch applied his postulates to identify the comma-shaped bacterium causing cholera outbreaks, linking it to the classic "rice water" diarrhea.
4. Peptic ulcer disease (Helicobacter pylori) A modern application: Barry Marshall and Robin Warren showed H. pylori was present in patients with gastritis/ulcers, isolated it in culture, and Marshall famously drank a culture of the organism himself, developed gastritis, and then recovered the organism from his own stomach - fulfilling all four postulates and overturning the belief that ulcers were caused by stress or acid alone.

Limitations (worth noting)

Koch himself recognized exceptions:
  • Asymptomatic carriers: Vibrio cholerae and Mycobacterium leprae (leprosy) can be found in healthy people too, violating postulate 1.
  • Viruses: many cannot be grown in pure culture on artificial media (postulate 2), and some (like poliovirus) cause disease in only a small fraction of infected hosts (postulate 1).
  • No suitable animal model: some pathogens (e.g., M. leprae) cannot be reproduced in standard lab animals, so postulate 3 cannot always be tested.
These gaps later led to refinements such as the molecular Koch's postulates (for genes/virulence factors) and the Bradford Hill criteria for causality in epidemiology.
A quick check of recent literature (PubMed) didn't surface any systematic reviews/meta-analyses that revise this classic teaching - it remains standard, uncontested material in microbiology and pathology education.
