Biomedical Waste (BMW) Management

(Based on Park's Textbook of Preventive and Social Medicine, 26th ed., and BMW Rules 2016, India)

1. Definition

According to the Bio-Medical Waste Management Rules 2016 (India):
"Bio-medical waste means any waste which is generated during the diagnosis, treatment or immunization of human beings or animals, or in research activities pertaining thereto, or in the production or testing of biologicals."

2. Significance

  • Health-care waste carries a higher potential for infection and injury than any other type of waste.
  • Inadequate handling leads to serious public health consequences and environmental impact.
  • BMW management is an integral feature of all health-care services.

3. Sources of BMW

All of the following are covered under BMW Rules:
  • Government/private hospitals, nursing homes, clinics
  • Primary health centres, dispensaries
  • Blood banks, mortuaries, slaughter houses
  • Medical research/training establishments
  • Laboratories, animal houses, biotechnology units
  • Vaccinating centres

5. Classification / Categories of BMW (Schedule I, BMW Rules 2016)

India's BMW Rules 2016 classify waste into 4 categories (revised from the older 10-category system):

6. Key Principles: The "SWEATA" / Waste Management Hierarchy

  1. Segregation - at the point of generation (most important step)
  2. Collection - in appropriate, labeled colour-coded containers
  3. Storage - not beyond 48 hours on-site
  4. Transportation - in closed, leak-proof vehicles; bar-coded
  5. Treatment - based on category (see below)
  6. Disposal - final safe disposal

8. BMW Rules 2016 - Key Provisions

  • Every healthcare facility must register with the State Pollution Control Board (SPCB).
  • Common Biomedical Waste Treatment Facilities (CBWTFs) are preferred over on-site treatment.
  • Mandatory bar-coding of waste containers for traceability.
  • Training of all health-care workers handling BMW is mandatory.
  • Occupational health protection (PPE, immunization of staff) is required.
  • Records must be maintained and submitted annually to SPCB.
  • Healthcare facilities with >500 beds must have their own treatment facility (incinerator/autoclave).

9. Universal Precautions in BMW Handling

  • All health-care workers must wear gloves, masks, and gowns while handling BMW.
  • Equipment must be washed with disinfectants after use.
  • BMW must be disinfected and disposed per BMW Management Rules 2016.
  • Needles must not be recapped, bent, or broken by hand (to prevent needlestick injuries).

  • Infectious waste: Waste that is suspected to contain pathogens in sufficient quantities to cause disease.
  • Sharps: Objects capable of causing cuts/punctures (needles, scalpels, blades, broken glass).
  • Pathological waste: Human/animal tissues, organs, body parts, blood.
  • Pharmaceutical waste: Expired/discarded drugs, vaccines, sera.
  • Cytotoxic waste: Highly hazardous waste from antineoplastic drug preparation/administration.
  • CBWTF: Common Bio-Medical Waste Treatment Facility - a centralized facility serving multiple healthcare institutions.
