Ab main aapko poora ECG padhne ka tarika simple Hinglish mein samjhaata hoon - textbooks aur diagram ke saath.
๐ซ ECG Kaise Padhein - Simple Guide (Hindi/Hinglish)
ECG Kya Hota Hai?
ECG (Electrocardiogram) dil ki electrical activity ko record karta hai. Dil jab bhi dhakta hai, ek electrical signal banta hai - wahi signal paper pe lines ke roop mein dikhta hai. Aap us line ko dekh ke samajh sakte hain dil theek hai ya nahi.
Pehle Samjho - ECG Ki Waves Kya Hain?
Yeh diagram dekho - ek normal ECG beat mein yeh parts hote hain:
| Wave/Segment | Matlab kya hai |
|---|
| P wave | Atria (upar ke chambers) ka electrical signal - atria contract karta hai |
| QRS complex | Ventricles (niche ke chambers) contract karte hain - dil ka main dhakna |
| T wave | Ventricles relax ho rahe hain (repolarization) |
| PR interval | P se QRS tak - AV node se signal guzarne ka time |
| ST segment | QRS ke baad T se pehle - ischemia ya heart attack dikhata hai |
| QT interval | Poore ventricular cycle ka time |

ECG Padhne Ka Systematic Tarika (14-Step Approach)
Harrison's Principles of Internal Medicine ke anusaar, hamesha ek order follow karo warna kuch miss ho jaata hai:
๐ข STEP 1 - Rate (Heart Rate Kitna Hai?)
Normal: 60-100 beats/minute
Fast calculate kaise karo:
- ECG paper pe ek bade square = 0.2 second
- 300 ko R waves ke beech bade squares se divide karo
- Example: R-R mein 3 bade boxes = 300/3 = 100 bpm
| Rate | Naam | Problem? |
|---|
| < 60 bpm | Bradycardia | Slow heart - thyroid, drugs, heart block |
| 60-100 bpm | Normal | โ
|
| > 100 bpm | Tachycardia | Fast heart - fever, anxiety, SVT, VT |
๐ข STEP 2 - Rhythm (Rhythm Regular Hai Ya Irregular?)
Dekho: Kya R-R interval equal hai?
- Regular + P wave pehle har QRS se = Normal Sinus Rhythm โ
- Irregular = Atrial Fibrillation (AF) ya arrhythmia
- P wave nahi hai = AF ya junctional rhythm
๐ข STEP 3 - P Wave Dekho
Normal P wave:
- Upright (positive) in Lead I aur Lead II
- Har QRS se pehle hona chahiye
- Duration: < 0.12 sec (3 small squares)
Abnormal kab:
- P wave nahi = AF
- P wave kabhi kabhi miss hoti = Heart Block
- P wave chodi ya peaked = Atrial enlargement
๐ข STEP 4 - PR Interval
Normal: 0.12 - 0.20 sec (3-5 small squares)
| PR Interval | Matlab |
|---|
| > 0.20 sec (lamba) | 1st Degree Heart Block |
| Increasingly lamba, phir QRS drop | 2nd Degree HB (Mobitz I) |
| Suddenly QRS drop | 2nd Degree HB (Mobitz II) |
| P aur QRS ka koi relation nahi | Complete (3rd Degree) Heart Block |
๐ข STEP 5 - QRS Complex
Normal: < 0.12 sec (< 3 small squares)
| QRS | Matlab |
|---|
| Choda (< 0.12 sec) | Normal |
| Choda (> 0.12 sec) | Bundle Branch Block (LBBB/RBBB) ya VT |
| Tall voltage | Ventricular Hypertrophy |
| Pathological Q wave | Old MI (purana heart attack) |
๐ข STEP 6 - ST Segment (SABSE IMPORTANT!)
Yeh segment normal mein FLAT hota hai (isoelectric line pe)
| ST Change | Matlab |
|---|
| ST Elevation | STEMI - Acute Heart Attack! ๐จ |
| ST Depression | NSTEMI / Ischemia / Digoxin effect |
| ST elevation + concave (saddle shape) | Pericarditis |
Yeh ek emergency sign hai - ST elevation dekho to turant action lo!
๐ข STEP 7 - T Wave
Normal: Upright (positive) Lead I, II, V4-V6 mein
| T Wave | Matlab |
|---|
| Inverted T wave | Ischemia, ventricular strain |
| Peaked/tall T wave | Hyperkalemia (high potassium) |
| Flat T wave | Hypokalemia, ischemia |
๐ข STEP 8 - QT Interval
Normal QTc: < 440 ms (men), < 460 ms (women)
- Long QT = Dangerous - risk of Torsades de Pointes (life-threatening arrhythmia)
- Causes: drugs (antidepressants, antiarrhythmics), hypokalemia, congenital
๐๏ธ Common ECG Problems - Quick Reference
| Condition | ECG Pe Kya Dikhega |
|---|
| Normal Sinus Rhythm | Regular, P before QRS, Rate 60-100 |
| Atrial Fibrillation (AF) | Irregular, no clear P waves, zigzag baseline |
| STEMI (Heart Attack) | ST elevation in regional leads |
| NSTEMI/Angina | ST depression, T wave inversion |
| 3rd Degree Heart Block | P aur QRS independent, heart rate slow |
| VT (V-Tach) | Wide QRS, fast rate, no P waves |
| VF (V-Fib) | Chaotic waves, no QRS - emergency! |
| LBBB | Wide QRS, M shape V5-V6, W shape V1 |
| RBBB | Wide QRS, RSR' ("rabbit ears") V1 |
๐ Yaad Rakhne Ka Formula (Mnemonic)
"Rate Rhythm P-wave PR QRS ST T QT"
Har ECG mein yahi order follow karo - ek bhi step skip mat karo!
Key Points - Ek Line Mein
- P wave = Atria ka signal (agar nahi = AF)
- QRS = Ventricle ka dhakna (wide = block ya VT)
- ST elevation = Heart attack ka sign (emergency!)
- Rate < 60 = Bradycardia, > 100 = Tachycardia
- Regular vs Irregular = Rhythm normal ya AF
Harrison's Principles of Internal Medicine (22nd Ed.) ke anusaar - "ECG interpretation mein systematic approach essential hai - mistakes aksar kisi cheez ko miss karne se hoti hain, isliye hamesha ek fixed order follow karo."
