ADA Standards of Care 2026 oral glucose tolerance test diagnostic criteria 75 g diabetes prediabetes

75-g OGTT values for nonpregnant adults
2-hour plasma glucoseInterpretation
<140 mg/dL (<7.8 mmol/L)Normal glucose tolerance
140-199 mg/dL (7.8-11.0 mmol/L)Prediabetes, specifically impaired glucose tolerance
≥200 mg/dL (≥11.1 mmol/L)Diabetes mellitus
For diagnosis in someone without clear symptoms of high glucose, an abnormal result generally needs confirmation with a repeat test or another diagnostic test. The ADA 2026 criteria use a 75-g glucose load with fasting and 2-hour samples.
Fasting sample, if included:
  • <100 mg/dL: normal
  • 100-125 mg/dL: impaired fasting glucose / prediabetes
  • ≥126 mg/dL: diabetes
Pregnancy OGTT cutoffs are different. Send the fasting, 1-hour, and 2-hour values plus whether this was done during pregnancy, and I can interpret them.
