Syrup Ondansetron — Pediatric Dose

Standard Dose

0.15 mg/kg PO — single oral dose (syrup or ODT)

Weight-Based Simplified Dosing (easier to use in practice)

WeightDose
8–15 kg2 mg
15–30 kg4 mg
>30 kg8 mg

Key Points

  • Indication: Vomiting in acute gastroenteritis, as adjunct to oral rehydration therapy (ORT)
  • Timing: Start ORT 15–30 minutes after giving ondansetron
  • Age: Most studies include children ≥6 months; some IV studies go down to 1 month
  • Single dose is sufficient — multiple doses add side effects without extra benefit
  • Route: PO preferred (syrup or ODT); IV use not routinely supported for this indication
  • Side effects: Mild, self-limited diarrhea; QT prolongation (~10–17 msec) — clinically insignificant in healthy children
  • Avoid dopamine antagonists (promethazine, metoclopramide) in children due to extrapyramidal reactions and respiratory depression risk
