Monitoring: serial serum bicarbonate/total CO₂, potassium, chloride, creatinine/eGFR, urine pH/citrate, and stone burden. The aim is normal acid-base status, normal potassium, prevention of stones, and normal growth in children. The
National Kidney Foundation overview similarly lists alkali agents as the main therapy.
Do not self-start potassium citrate if there is impaired kidney function, hyperkalemia, or use of drugs that raise potassium, such as ACE inhibitors, ARBs, spironolactone, or trimethoprim, without clinician supervision.
