Pathophysiological Mechanisms

1. Corticospinal Tract (Lateral) Compression

The lateral corticospinal tract runs in the lateral funiculus and carries voluntary motor commands to the hand. Compression (from disc herniation, osteophytes, ligamentum flavum hypertrophy, or dynamic stenosis) disrupts:
  • Fine motor control — difficulty with keyboard typing, writing, buttoning shirts, using keys
  • Rapid alternating movements — impaired grip-and-release cycling
  • Spastic co-contraction — muscles cannot smoothly transition between flexion and extension
The corticospinal tracts are somatotopically arranged, with cervical fibers in the most central/medial position. Cord compression at C3–C7 selectively injures these fibers early.

2. Anterior Horn Cell Damage (Lower Motor Neuron Component)

Compression also injures the anterior horn cells of the cervical enlargement, which supply the intrinsic hand muscles (interossei, lumbricals, thenar/hypothenar muscles):
  • Intrinsic muscle wasting — atrophy of interossei and hypothenar muscles
  • The combination of UMN spasticity + LMN wasting gives the myelopathy hand its mixed picture

3. Posterior Column (Dorsal Column) Dysfunction

Compression of the dorsal columns disrupts proprioception and fine-touch discrimination:
  • Impaired joint position sense in the fingers
  • Contributes to clumsiness even when motor strength is relatively preserved
  • Patients struggle with tasks requiring sensory feedback (e.g., doing up buttons without looking)

4. Vascular Ischemia

Dynamic and static compression reduces blood flow in the pial arterial plexus and penetrating arteries supplying the cord gray matter:
