I read this directly from the photo, and the handwriting is hard. Some names are clear and some are only my best guess. Please confirm the uncertain ones with the clinic or a pharmacist before taking anything.
Header
  • Date: 26/10/26
  • Patient: V. Kamaladevi, 75/F, from Palasa
  • BP, DM and thyroid: all marked negative (⊖)
  • Clinic: Reva Spine Centre, Dr. Arun Kumar V (spine surgeon)
Diagnosis and complaints
  • P/o L4-L5 fusion (post-operative, after fusion of the L4-L5 spine levels).
  • C/o radiculopathy in the lower limb (nerve pain, probably "B/L LL", meaning both legs), plus buttock pain.
Medicines, written as Tab. (T.)
#As writtenWhat it most likely isDose and timing
1T. GABAPENTINGabapentin 400 mg, a nerve pain medicine. This one is clear.1-0-1 (morning and night)
2T. "NERVICORT" / "HBRUICORT" (unclear)Probably a nerve or vitamin type tablet, 75 mg. I can't read the name reliably.1-0-1
3T. "THURGWIRE" (unclear)I can't identify it from the handwriting.0-0-1 (night only)
4T. SOOTHAR-PProbably a pain relief combination tablet with paracetamol ("-P"). A word beside it looks like "Ultra..." and may be an alternative or an SOS note.1-0-1
5"…MYCIN" powder (the first letters are unclear)An antibiotic dusting powder, "for L/A" (local application). It is probably for the surgical wound.Apply locally
  • The bracket on the right of items 1 to 3 appears to read 400 mg / 75 mg / 6 weeks. That suggests items 1 to 3 are meant for about 6 weeks, though I can't be sure.
  • The dose pattern 1-0-1 means one tablet in the morning, none at noon, and one at night. 0-0-1 means night only.
Other advice on the left
  • Wear an L.S. belt (lumbosacral belt).
  • Physiotherapy: IFT, TENS and US (interferential therapy, TENS and ultrasound) for the lower limbs.
What to do next
  • Show the paper to a pharmacist, or call the clinic on 720 7120 620, to confirm items 2, 3, 4 and 5.
  • Ask the pharmacist to read the names aloud and match them to the doctor's intent.
  • The patient is 75, so ask about dizziness and sedation from gabapentin and any other nerve pain tablets.
I tried the image analysis tool, but it couldn't open the uploaded file. If you send a closer, straight-on photo of just the medicine section, I can try to read items 2, 3, 4 and 5 more reliably.
