NT-proBNP is a blood marker released when the heart muscle, especially the lower pumping chambers, is stretched or working under high pressure.
Why it rises
If the heart cannot pump or fill normally, pressure builds up inside the heart. The heart releases NT-proBNP in response. Therefore, a high result can be a sign of heart failure or fluid/pressure overload.
It is most useful when someone has symptoms such as:
- Shortness of breath, especially lying flat or on exertion
- Leg or ankle swelling
- Rapid weight gain from fluid
- Tiredness
- Waking at night breathless
It helps support the diagnosis, but
it cannot diagnose heart failure alone. Clinicians interpret it with symptoms, examination, ECG, chest imaging, kidney tests, and an
echocardiogram. It also cannot tell whether heart failure has preserved or reduced ejection fraction, as noted in
NICE recommendations.
Why it may be elevated without heart failure
NT-proBNP may rise with:
- Reduced kidney function
- Atrial fibrillation or other rapid rhythms
- Increasing age
- Heart attack, valve disease, or pulmonary hypertension
- Pulmonary embolism, severe pneumonia, sepsis, or critical illness
Kidney impairment is particularly important: NT-proBNP can accumulate when renal clearance is reduced, so the number may be high even without acute fluid overload. What the numbers commonly mean
For suspected chronic heart failure in an untreated adult, NICE uses these referral thresholds:

These are assessment thresholds, not a diagnosis or a measure of immediate danger in isolation.
NICE guidance also notes that very high levels are associated with poorer prognosis.
Simple summary
Think of it as a “heart strain” signal:
- Low level: heart failure less likely.
- High level: heart failure becomes more likely, but other causes must be considered.
- Very high level plus breathlessness/swelling: needs prompt medical assessment.
Seek emergency help now for severe or rapidly worsening breathlessness, chest pain, fainting, confusion, blue/grey lips, or inability to lie flat due to breathlessness.
