atorvastatin rosuvastatin prescribing information LDL lowering dose comparison FDA

Both are highly effective statins. Rosuvastatin is more potent milligram-for-milligram, while atorvastatin has fewer renal dosing constraints but more CYP3A4 drug-interaction issues.

Common brandLipitorCrestor
Moderate-intensity dose10-20 mg daily5-10 mg daily
High-intensity dose40-80 mg daily20-40 mg daily
LDL lowering at comparable dosesVery strongUsually slightly stronger
MetabolismMainly CYP3A4Minimal CYP metabolism, mainly transporter-mediated
Kidney impairmentNo usual dose adjustmentIf severe renal impairment and not on dialysis: generally start 5 mg, max 10 mg/day

Which is “better”?
  • For the largest LDL reduction with a relatively low tablet dose, rosuvastatin is often chosen.
  • If someone has significant chronic kidney disease, atorvastatin is often simpler because it does not usually require renal dose adjustment.
  • If someone takes medicines that strongly inhibit CYP3A4, such as certain macrolide antibiotics, antifungals, HIV medications, or cyclosporine, the choice and dose need extra care. Atorvastatin is particularly affected by CYP3A4 inhibitors.
  • At equivalent guideline intensity, both reduce cardiovascular risk substantially. The choice is usually based on LDL target, tolerance, kidney function, cost/formulary, and concomitant drugs.
Side effects Both can cause muscle aches, small liver-enzyme rises, and a small increase in diabetes risk in predisposed people. Severe muscle injury is rare, but urgent assessment is warranted for severe muscle pain/weakness with dark urine, especially during an interacting medication or acute illness.
A recent 2025 systematic review compared their effects on lipid levels. The practical takeaway remains dose equivalence rather than assuming one is universally superior.
Do not switch doses 1:1 without prescriber guidance. A common approximate intensity match is atorvastatin 20 mg ≈ rosuvastatin 10 mg, and atorvastatin 40-80 mg ≈ rosuvastatin 20-40 mg.
