Here is the pediatric dosing for a 4-month-old infant for both drugs:

Paracetamol (Acetaminophen) - 4-Month-Old Infant

A 4-month-old typically weighs around 6-7 kg (varies by growth chart).
ParameterDose
Oral dose10-15 mg/kg/dose every 4-6 hours
Rectal (PR) dose10-20 mg/kg/dose every 4-6 hours
Max daily dose60 mg/kg/day (for children under 2 years)
Max doses per day5 doses in 24 hours
FormulationSuspension 160 mg/5 mL (paediatric syrup)
Practical example for a 6.5 kg baby:
  • Dose = 10-15 mg/kg x 6.5 kg = 65-97.5 mg per dose
  • Using 160 mg/5 mL syrup = approximately 2-3 mL per dose every 4-6 hours

Metronidazole - 4-Month-Old Infant

Metronidazole is used from 2 months of age onwards. At 4 months, the dosing is weight-based:

Anaerobic/bacterial infections7.5-10 mg/kg/doseEvery 6-8 hours
Amebiasis35-50 mg/kg/day divided in 3 dosesFor 10 days
Giardiasis / GI infection15 mg/kg/day divided TDS (max 250 mg/dose)5-7 days
IV (serious anaerobic infections)7.5 mg/kg IVEvery 6-8 hours
Max single dose500 mg-
Practical example for a 6.5 kg baby:
  • Dose = 7.5 mg/kg x 6.5 kg = ~49 mg per dose every 8 hours
  • Oral metronidazole suspension 200 mg/5 mL = approximately 1.2 mL per dose

Important Safety Notes

  • Paracetamol under 2 months: Use only under medical supervision; dosing is different for neonates.
  • Metronidazole under 2 months: Generally avoided unless strictly necessary; pharmacokinetics are unpredictable in very young infants.
  • Always calculate based on actual body weight - obtain an accurate weight before dosing.
  • Do not exceed maximum daily doses - paracetamol overdose causes serious liver toxicity.
  • Metronidazole may cause GI upset; give with or after feeds to minimise this.
  • Consult a paediatrician or pharmacist before administering to confirm indication and exact dose for the individual baby.
