Pioglitazone is a thiazolidinedione (TZD) that acts as an agonist of the nuclear peroxisome proliferator-activated receptor-gamma (PPAR-gamma).

Mechanism

  1. It enters cells and binds PPAR-gamma, mainly in adipose tissue, skeletal muscle, and liver.
  2. The activated receptor changes transcription of insulin-responsive genes involved in glucose and lipid handling.
  3. This reduces insulin resistance:
    • Increases insulin-mediated glucose uptake in skeletal muscle and adipose tissue
    • Reduces hepatic gluconeogenesis, lowering hepatic glucose output
    • Alters adipokines, including increased adiponectin, which supports insulin sensitivity

Key point

It does not stimulate pancreatic insulin release. Therefore, it requires endogenous insulin to work and, when used alone, has a relatively low risk of hypoglycemia.
Net effect: lower blood glucose by improving peripheral and hepatic insulin sensitivity. The FDA label description similarly states that it reduces peripheral and hepatic insulin resistance.
