Definition & Pathology

GPA is an idiopathic, necrotizing granulomatous vasculitis affecting small and medium-sized vessels, characterized by a classic triad (Godman and Churg criteria):

Pathogenesis

Clinical Features

Upper Respiratory Tract (90% - most common initial presentation)

  • Persistent rhinorrhea, severe sinusitis, nasal mucosal ulceration
  • Epistaxis, nasal septal perforation
  • Failure to respond to conventional sinusitis treatment is a diagnostic clue
  • Combined nasal and palatal destruction → saddle-nose deformity
  • Oral: "strawberry gums" (hypertrophic granulomatous gingivitis - pathognomonic)
  • Palatal/tongue ulceration, subglottic stenosis

Pulmonary (71% have infiltrates radiographically)

  • Cough, dyspnea, chest pain
  • Nodules, cavities, pulmonary infiltrates, hemoptysis

Renal (85%)

  • Focal crescentic necrotizing glomerulonephritis
  • Hematuria, proteinuria, AKI → renal failure (historically the most common cause of death)

Ear

  • Present in 19-56% of patients
  • Conductive hearing loss, sensorineural hearing loss, tinnitus, vertigo, otalgia
  • Draining tympanic membrane perforation; medically refractory chronic otitis media (mimics TB but with disproportionate pain)
  • Facial nerve paresis

Skin (45%)

  • Palpable purpura, nodules (especially extensor surfaces), ulcers
  • Necrotizing angiitis → hemorrhagic pustular eruption, livedo reticularis
  • Pyoderma gangrenosum-like lesions

Other Systems

  • Eyes (58%): conjunctivitis, episcleritis, proptosis, orbital pseudotumor

Diagnosis

ACR/EULAR Criteria (paediatric: 3 of 6 features required)

  1. Renal involvement
  2. Upper airway involvement
  3. Pulmonary involvement (infiltrates, nodules, or cavities)
  4. Laryngo-tracheobronchial stenosis
  5. Granulomatous inflammation on biopsy
  6. ANCA positivity

Key Tests

  • c-ANCA / PR3-ANCA: highly specific; positive in ~90% generalized, ~70-90% limited forms (up to 30% can be negative, especially limited disease)
  • Biopsy: sinonasal biopsies have the highest specificity and sensitivity; ear biopsies often show only non-specific inflammation
  • Histology: necrotizing granulomas + vasculitis; LCV pattern in skin; crescents on renal biopsy
  • Urinalysis, creatinine, CXR/CT chest, ESR/CRP

Treatment

Induction of Remission

  • Plasma exchange (PLEX): for diffuse pulmonary hemorrhage or RPGN near dialysis
  • IVIG, anti-TNF therapy (infliximab), tacrolimus

Newer Agents

  • Avacopan (C5a receptor blocker): steroid-sparing agent used alongside CYC or rituximab

Prognosis

  • Untreated: mean survival 5 months, 90% mortality at 2 years
  • With cyclophosphamide: complete remission in up to 93%, lasting average 4 years
  • Limited form (upper respiratory tract only, no renal involvement): better prognosis
  • Therapy-related adverse events (CYC toxicity, infection) now account for more deaths in year 1 than the vasculitis itself

Recent Guidelines

The 2025 British Society for Rheumatology guidelines for ANCA-associated vasculitis (PMID 40499922) represent the most current practice framework. A 2025 meta-analysis also examined alpha-1 antitrypsin deficiency as a genetic risk factor for GPA (PMID 41125409), reinforcing the role of SERPINA1 variants in pathogenesis.
