Boil (Furuncle) in the Ear — Recommended Ear Drops

A small boil in the ear is called a furuncle of the external auditory canal, a form of localized (acute bacterial) otitis externa, typically caused by Staphylococcus aureus infecting a hair follicle in the outer third of the ear canal.

First-line choice: Ciprofloxacin ear drops (with or without a corticosteroid like hydrocortisone) — per Bailey & Love's Surgery, 28th Ed. (p. 776) and general ENT guidelines.

How to Use

  1. Clean the ear canal gently before instilling drops.
  2. Instill 3–4 drops, 2–3 times daily for 7–10 days.
  3. Lie on the opposite side for 2–3 minutes after instilling to let the drops reach the site.
  4. A wick (Pope wick) may be placed by a doctor if the canal is swollen shut, to carry the drops to the site of infection.

Additional Management

  • Analgesics (ibuprofen/paracetamol) for pain relief.
  • Do NOT insert cotton buds or attempt to squeeze the boil — this worsens infection.
  • If the furuncle is large, pointing, or fluctuant, incision and drainage by an ENT specialist is needed.
  • Oral antibiotics (e.g., flucloxacillin or co-amoxiclav) are reserved for:
    • Spreading cellulitis of the outer ear (pinna)
    • Immunocompromised patients
    • Systemic signs (fever, lymphadenopathy)

When to See a Doctor Urgently

  • Severe or worsening pain
  • Swelling spreading to the face/jaw
  • Fever
  • Hearing loss
  • Diabetes or immunosuppression (risk of necrotizing otitis externa)
