"brivaracetam"[MeSH Terms] OR brivaracetam

Both are anti-seizure medicines. I'm reading the names as brand names, since "Bervipil" matched "Brevipil" and "Diva od" matched "Divaa-OD". Please check the spelling on your strips.

Divaa-OD 250 (divalproex sodium ER, 250 mg, once daily)

This is an extended-release tablet of divalproex, a form of valproate. It is used for:
  • Epilepsy. It treats complex partial seizures and absence seizures, either alone or with other drugs, and also multiple seizure types that include absence seizures.
  • Acute manic episodes in bipolar disorder.
  • Migraine prevention.
The extended-release form is approved for seizures, acute mania and migraine. Divalproex also has indications for migraine prophylaxis and bipolar mania in addition to epilepsy (Kaplan & Sadock's Comprehensive Textbook of Psychiatry). Tintinalli's Emergency Medicine lists valproate for tonic-clonic, absence and complex partial seizures, and for posttraumatic epilepsy.
Key cautions:
  • Do not stop it suddenly, because that can trigger seizures or mood changes.
  • Swallow it whole.
  • It carries a risk of liver damage and pancreatitis.
  • It causes major birth defects, so it is generally avoided in pregnancy and in women who could become pregnant unless there is no alternative.
  • Common effects are drowsiness, nausea, tremor, weight gain and hair thinning.
  • Liver function and blood counts are monitored.

Brevipil 50 (brivaracetam, 50 mg)

This is an add-on or single-drug treatment for focal (partial-onset) seizures in people with epilepsy. Katzung's Basic and Clinical Pharmacology (16th ed.) describes brivaracetam as the 4-n-propyl analog of levetiracetam. It binds the synaptic vesicle protein SV2A with high affinity.
The same book says there is no evidence it is more effective than levetiracetam. It also gives no added benefit when combined with levetiracetam, because both drugs act on SV2A. It can raise phenytoin levels.
Common effects: sleepiness, dizziness, fatigue, nausea and vomiting. Avoid driving until you know how it affects you. Mood or behaviour changes, anxiety, depression and suicidal thoughts need prompt medical advice. Rarely, it causes serious allergic reactions.

Taken together

Your doctor has probably prescribed them together for epilepsy that needs more than one drug. Brivaracetam is usually added to a drug like valproate when seizures are not fully controlled. The dose and timing are set by your doctor, so don't change or stop either one on your own.
Recent evidence: Two recent reviews support brivaracetam for focal epilepsy (PMID 42335525, PMID 40319125 in children). I found nothing that contradicts the textbook description.
If you tell me the condition you're being treated for, I can be more specific. This is general information and not a substitute for your prescriber's advice.
