For scrub typhus, doxycycline is typically:
  • Adults and children weighing 45 kg or more: 100 mg orally or IV every 12 hours
  • Children under 45 kg: 2.2 mg/kg per dose orally or IV every 12 hours
    Maximum 100 mg per dose
Continue until the patient has been afebrile for at least 3 days and is clinically improving. A minimum 5 to 7-day course is generally used; many scrub-typhus regimens use 7 days. Do not use a single dose or very short course because relapse may occur. The CDC scrub typhus guidance recommends doxycycline for suspected infection at all ages.
For pregnancy or a serious doxycycline allergy, treatment should be chosen with an infectious-disease clinician, often using an alternative such as azithromycin.
