Key Clinical Points

  1. Frankenhäuser (uterovaginal) plexus - the final relay ganglion lying lateral to the cervix within the cardinal and uterosacral ligaments. It is the target of paracervical block for labor analgesia. (Color Atlas of Human Anatomy Vol. 2; Berek & Novak's Gynecology)
  2. Presacral neurectomy - resects the superior hypogastric plexus to interrupt uterine/cervical afferents, relieving dysmenorrhea in 50–75% of endometriosis cases. Adnexal pain is NOT relieved since ovarian fibers travel with the ovarian plexus. (Berek & Novak's Gynecology)
  3. Upper vagina/cervix/uterus receive separate innervation from lower thoracic nerves - pudendal nerve block alone is insufficient for uterine manipulation. (Pfenninger & Fowler's Procedures for Primary Care)
  4. Radical hysterectomy disrupts the inferior hypogastric plexus and uterovaginal plexus, causing post-operative bladder, bowel, and sexual dysfunction. (Berek & Novak's Gynecology)
  5. Referred pain from the uterus/cervix is felt in the lower abdomen and high lumbar spine (via T10–L1). Because the uterus shares innervation with the sigmoid colon and rectum, GI and gynecological pain can be clinically indistinguishable. (Berek & Novak's Gynecology)
