When to deliver the fetus in various diseases complicating pregnancy

Reading File
This is a broad obstetric-management topic. Timing of delivery varies with maternal stability, fetal status, gestational age, severity of disease, and local neonatal facilities.
  1. Hypertensive disorders
    • Chronic hypertension
    • Gestational hypertension
    • Preeclampsia with or without severe features
    • Eclampsia, HELLP syndrome
  2. Diabetes in pregnancy
    • Diet-controlled gestational diabetes
    • Medication-controlled gestational diabetes
    • Pregestational type 1 or type 2 diabetes
    • Diabetes with vascular disease or poor glycemic control
  3. Cardiac disease
    • Valvular lesions
    • Cardiomyopathy or heart failure
    • Pulmonary hypertension
    • Arrhythmias and anticoagulation issues
  4. Renal and autoimmune disease
    • Chronic kidney disease
    • Systemic lupus erythematosus
    • Antiphospholipid syndrome
  5. Hepatic and hematologic disorders
    • Intrahepatic cholestasis of pregnancy
    • Acute fatty liver of pregnancy
    • Viral hepatitis
    • Thrombocytopenia and inherited bleeding disorders
  6. Other major medical conditions
    • Epilepsy
    • Thyroid disease
    • Asthma
    • HIV and other infections
    • Malignancy
  7. Obstetric conditions that commonly determine delivery timing
    • Fetal growth restriction
    • Oligohydramnios
    • Placenta previa/accreta
    • Prelabor rupture of membranes
    • Multiple pregnancy
    • Rh alloimmunization
High-level principle: deliver immediately for uncontrolled severe maternal disease, eclampsia/HELLP, major hemorrhage, acute fetal compromise, or conditions in which continuing pregnancy is more dangerous than prematurity. In stable disease, surveillance is used to safely reach a disease-specific planned gestational age.

Delivery timing in diseases complicating pregnancy

These are commonly used ACOG/SMFM-based timings for a singleton pregnancy with reliable dating. “Deliver” means plan induction or cesarean as clinically appropriate. The route of delivery depends on obstetric indications, not gestational age alone. Recommendations must be individualized for maternal deterioration, abnormal fetal surveillance, fetal growth restriction (FGR), abruption, or labor.
Disease/conditionClinical statusRecommended timing of delivery
Chronic hypertensionUncomplicated, not requiring medication38+0 to 39+6 weeks
Controlled on antihypertensive medication37+0 to 39+6 weeks
Difficult to control36+0 to 37+6 weeks
Gestational hypertensionNo severe-range blood pressure37+0 weeks, or at diagnosis if later
Severe-range BP34+0 weeks, or at diagnosis if later
PreeclampsiaWithout severe features37+0 weeks, or at diagnosis if later
With severe features, mother and fetus stable34+0 weeks, or at diagnosis if later
Eclampsia, HELLP syndrome, uncontrolled severe hypertension, pulmonary edema, renal/hepatic deterioration, abruption, or nonreassuring fetusExpedite delivery after maternal stabilization, regardless of gestational age
Chronic hypertension with superimposed preeclampsiaWithout severe featuresUsually 37+0 weeks
With severe features34+0 weeks if stable; earlier if unstable
Gestational diabetes mellitus (GDM)Diet controlled, good control (A1GDM)39+0 to 40+6 weeks
Medication controlled, good control (A2GDM)39+0 to 39+6 weeks
Poor control or vascular complicationsIndividualize, often 37+0 to 38+6 weeks depending on severity and fetal testing
Pregestational type 1 or type 2 diabetesWell controlled, no vascular disease39+0 to 39+6 weeks
Vasculopathy, nephropathy, poor glycemic control, prior stillbirth, or other complications36+0 to 38+6 weeks, individualized
Intrahepatic cholestasis of pregnancyTotal bile acids <100 micromol/L36+0 to 39+0 weeks. Lower bile-acid levels can generally be managed nearer 39 weeks.
Total bile acids ≥100 micromol/L36+0 weeks
Severe symptoms, worsening liver tests, prior ICP-related stillbirth, or other complicationsConsider earlier, individualized
Acute fatty liver of pregnancyAny confirmed/suspected AFLPImmediate delivery after maternal stabilization
HELLP syndromeAny gestation with maternal or fetal instabilityImmediate/expedited delivery after stabilization
Chronic kidney disease / renal insufficiencyStable renal function, controlled BP, normal fetal growthNo fixed disease-only timing. Usually aim for 39 weeks if uncomplicated.
Worsening renal function, uncontrolled hypertension, superimposed preeclampsia, FGR, or abnormal testingIndividualize and deliver earlier for maternal or fetal indication
Systemic lupus erythematosusQuiescent disease, normal renal function, no obstetric complicationNo routine early delivery. Aim for 39 weeks if stable.
Active nephritis, severe flare, hypertension/preeclampsia, FGR, or abnormal fetal surveillanceIndividualize, often earlier based on the complication
Antiphospholipid syndromeStable on anticoagulation, no placental diseaseUsually 39 weeks; coordinate stopping/bridging anticoagulation around planned delivery.
Prior thrombosis, severe placental insufficiency, FGR, preeclampsia, abruptionIndividualize
Cardiac diseaseStable valvular, congenital, or arrhythmic disease without heart failureUsually 39 weeks, preferably planned vaginal delivery unless a cardiac/obstetric reason requires cesarean.
Decompensated heart failure, severe valvular obstruction, aortic dissection risk, pulmonary hypertension, worsening cyanosisIndividualized early delivery in a tertiary center after multidisciplinary assessment
Pulmonary hypertension / Eisenmenger syndromeSignificant diseaseTiming and route are highly individualized in a tertiary center. Delivery is often planned around 34-36 weeks if stable, or earlier for deterioration.
AsthmaWell controlledNo indication for early delivery. Aim for routine obstetric timing, usually 39 weeks.
EpilepsyControlledNo indication for early delivery. Aim for routine timing.
Thyroid diseaseControlled hypo- or hyperthyroidismNo indication for early delivery.
Thyroid storm or severe uncontrolled diseaseStabilize urgently; delivery only for maternal/fetal or obstetric indication.
HIVViral load >1,000 copies/mL or unknown near deliveryPlanned cesarean at 38+0 weeks
Viral load ≤1,000 copies/mL on effective ARTVaginal delivery is appropriate; deliver for usual obstetric indications, generally not before 39 weeks solely because of HIV.
Active genital herpesLesions or prodromal symptoms at laborCesarean at onset of labor/rupture of membranes, rather than delivery at a fixed week.
Hepatitis B or CUncomplicated infectionNo routine early delivery and no cesarean solely to prevent transmission.

Major fetal, placental, and obstetric indications commonly tested with medical disease

ConditionSuggested delivery timing
FGR, estimated fetal weight 3rd-10th centile, normal umbilical artery Doppler38+0 to 39+0 weeks
Severe FGR, EFW <3rd centile37+0 weeks
FGR with decreased diastolic flow in umbilical artery37+0 to 37+6 weeks
FGR with absent end-diastolic flow33+0 to 34+0 weeks
FGR with reversed end-diastolic flow30+0 to 32+0 weeks
Isolated oligohydramnios36+0 to 37+6 weeks, or at diagnosis if later
PPROM34+0 to 36+6 weeks if no earlier indication such as infection, labor, abruption, or fetal compromise
Term PROMDelivery at diagnosis, usually induction
Placenta previa, uncomplicated36+0 to 37+6 weeks
Placenta accreta spectrum, uncomplicated34+0 to 35+6 weeks in an experienced center
Vasa previa34+0 to 37+0 weeks by cesarean
Monochorionic-diamniotic twins, uncomplicated34+0 to 37+6 weeks
Dichorionic-diamniotic twins, uncomplicated38+0 to 38+6 weeks

Deliver irrespective of the planned gestational age if there is

  • Eclampsia, HELLP syndrome, acute fatty liver, or severe uncontrolled preeclampsia
  • Maternal pulmonary edema, stroke, myocardial decompensation, or progressive renal/hepatic failure
  • Placental abruption or major antepartum hemorrhage
  • Chorioamnionitis
  • Nonreassuring fetal status, fetal demise, or severe Doppler deterioration
  • Persistent severe hypertension despite treatment
The ACOG/SMFM delivery-timing guidance emphasizes that a genuine indication should not be deferred merely to reach 39 weeks or to complete fetal lung-maturity testing. For cholestasis, the bile-acid threshold and timing reflect SMFM ICP guidance.
