Answer: Do NOT give Loperamide

Loperamide is contraindicated in this patient. Your textbooks are clear on this:
"Antimotility medications such as loperamide may decrease illness duration for mild to moderate nonbloody diarrhea in adults without fever; avoid these in children and patients with dysentery (fever and bloody diarrhea) due to concerns of prolonging the illness."

Why it's dangerous here:

  1. Bloody diarrhea = invasive organism (likely Shigella, Campylobacter, EIEC, or C. difficile in elderly)
  2. Loperamide slows motility - this traps the organism and toxins in the gut, prolonging mucosal damage
  3. Risk of toxic megacolon - especially in a 65-year-old
  4. In E. coli O157:H7, it may increase risk of HUS (hemolytic uremic syndrome)

  • Monitor electrolytes, creatinine, CBC - 65-year-old with bloody diarrhea can deteriorate fast
  • Stool frequency charting - if not improving in 48 hrs on metro + cipro, reconsider organism (culture result will guide)
  • Do NOT use loperamide unless diarrhea becomes confirmed nonbloody and infective cause cleared
Keep the current regimen. Skip the loperamide.
