Yes. Nifedipine 20 mg and telmisartan 40 mg are often prescribed together for high blood pressure. They lower blood pressure by different mechanisms, but the combination can sometimes lower it too much.
- Take them exactly as your clinician prescribed. They may be taken at the same time or at different times of day.
- Watch for dizziness, light-headedness, fainting, unusual weakness, or blurred vision, especially when standing up. Sit or lie down if this occurs and rise slowly.
- Monitor your blood pressure, particularly during the first few days or after dose changes.
- Avoid grapefruit/grapefruit juice with nifedipine because it can raise nifedipine levels.
- Telmisartan must not be used in pregnancy. Seek medical advice promptly if pregnant or planning pregnancy.
- If you have kidney disease, dehydration from vomiting/diarrhea, or take potassium supplements, salt substitutes, NSAID pain medicines such as ibuprofen, or other BP medicines, ask your prescriber or pharmacist for individualized advice.
Telmisartan 40 mg is a standard starting antihypertensive dose according to DailyMed prescribing information. The main concern with combining BP medicines is additive low blood pressure, so monitoring is appropriate.

Do not take nifedipine 20 mg four times a day unless the prescriber specifically wrote that schedule. That would be 80 mg/day, and whether it is appropriate depends entirely on the formulation and reason for treatment.
- Extended-release / sustained-release 20 mg tablets: usually taken once or twice daily, not four times daily. Do not crush, split, or chew them.
- Immediate-release 20 mg capsules/tablets: can sometimes be prescribed multiple times per day in selected situations, but this is less commonly used for routine high blood pressure because it may cause rapid BP drops, fast heartbeat, headache, flushing, or dizziness.
- With telmisartan 40 mg, four daily doses may increase the chance of excessively low blood pressure.
Check the packet: does it say ER, XL, SR, CR, retard, extended-release, or sustained-release? If yes, do not take it 4 times daily without urgent confirmation from your doctor/pharmacist.
If you already took extra doses and have fainting, severe dizziness, chest pain, shortness of breath, or a very low BP, seek urgent medical care. DailyMed's nifedipine ER label describes usual ER maintenance dosing as 30 to 60 mg once daily, with doses above 90 mg/day generally not recommended.
