LISA "less invasive surfactant administration" neonatal

Reading File
In surfactant therapy, LISA stands for:

LISA = Less Invasive Surfactant Administration

It is a technique for delivering surfactant to preterm infants with - or at risk of - Respiratory Distress Syndrome (RDS) without endotracheal intubation.

How it works

  • A thin, soft catheter (e.g., a vascular catheter or feeding tube) is passed into the trachea of a spontaneously breathing infant
  • Surfactant is instilled through the catheter while the infant remains on nasal CPAP (nCPAP)
  • The catheter is then removed; CPAP is maintained throughout

Why LISA over conventional intubation?

FeatureConventional (INSURE)LISA
Ventilation during surfactantEndotracheal tube + PPVThin catheter only; infant breathes spontaneously
CPAP interruptionYesMinimal/none
Risk of ventilator-induced lung injuryHigherLower
Surfactant distributionGoodGood (spontaneous breathing aids distribution)
BPD riskHigherReduced

Clinical context

LISA combines the benefits of CPAP (preventing atelectasis, preserving surfactant function, avoiding intubation) with the benefit of exogenous surfactant replacement. It has been increasingly adopted in Europe and Australia and is now supported by multiple systematic reviews and meta-analyses.
Recent evidence (2023-2026) confirms its safety and efficacy:
