What It Is

Steatocystoma multiplex is a benign hamartomatous condition of the pilosebaceous unit, characterized by numerous sebaceous duct cysts. It is caused by mutations in the KRT17 gene (keratin 17), which can be inherited in an autosomal dominant pattern or occur sporadically.

Management

SM is benign and treatment is elective (cosmetic or symptomatic):

  • Radiofrequency ablation / CO₂ laser — good cosmetic outcome, especially for scrotal lesions
  • Retinoids (oral isotretinoin) — may reduce new cyst formation, doesn't resolve existing cysts
  • No treatment required if asymptomatic

The clinical picture here — numerous smooth, round, dome-shaped, tan/skin-colored cysts scattered over the scrotum in a young male — is essentially diagnostic of steatocystoma multiplex. Biopsy confirms if uncertain.
