Sites to Check Pallor

Pallor is best assessed at sites where the skin is thin, pigmentation is minimal, and mucous membranes or superficial vasculature is easily visible. It can be difficult to detect in patients with naturally dark skin pigmentation if only the skin is checked - that is why mucous membranes are preferred.

Why Conjunctiva and Palmar Creases Are Preferred

As noted in Goldman-Cecil Medicine: "Pallor of the conjunctival mucosa or of the creases in the palm are more specific than pallor in other sites." This is because:
  • These sites are free from melanin pigmentation
  • The mucosa/vasculature is superficial and directly visible
  • They are not affected by peripheral vasoconstriction as much as the skin
