Types in Detail

1. Simple (Non-Toxic) Goitre

A diffuse or nodular enlargement in a euthyroid patient. The most common cause globally is iodine deficiency (daily requirement: ~0.1-0.15 mg/day). Endemic areas include mountain ranges (Alps, Andes, Himalayas, Rocky Mountains) and inland lowlands far from sea (Great Lakes, Congo, Nile Valley, Derbyshire UK). Calcium is also goitrogenic - goitre is more common in chalk/limestone areas with low iodine.
Goitrogenic foods and drugs (goitrogens) that interfere with hormone synthesis can also cause simple goitre when iodine intake is marginal.

2. Multinodular Goitre (MNG)

Nodules appear early in endemic goitre but later (age 20-30 years) in sporadic goitre. The patient may be unaware until their 40s or 50s. All forms of simple goitre are more common in females due to oestrogen receptors in thyroid tissue.

3. Toxic Goitre

  • Graves' disease: goitre occurs in nearly 100% of patients. It is one of the five hallmarks - the others being thyrotoxicosis, ophthalmopathy, pretibial myxedema, and acropachy (clubbing).
  • Toxic MNG: episodes of mild hyperthyroidism occur in up to 30% of patients with MNG.

4. Retrosternal (Substernal) Goitre

Arises from slow inferior growth of a multinodular gland into the mediastinum. Pressure within the thoracic inlet causes dysphagia, tracheal compression, and airway symptoms. Clinical Features

Complications

  • Tracheal obstruction - lateral displacement or anteroposterior compression, especially with retrosternal extension
  • Acute airway compromise - haemorrhage into a nodule impacted at the thoracic inlet

Treatment

Prevention

Medical

  • Early hyperplastic goitre: thyroxine 0.15-0.2 mg daily for several months may cause regression.
