Use Pulsatilla here because: Thirstlessness + bland creamy leucorrhea + migratory joint pains are present together — this triad is pathognomonic of Pulsatilla and cannot be explained by Sepia. The prolapse is covered by Pulsatilla as well, just not as its first keynote.
Sepia may still be needed as an intercurrent or follow-up remedy after Pulsatilla acts, especially to consolidate the prolapse and hormonal picture — but it is not the simillimum in this case.
