If by ISOP you mean isoprenaline (isoproterenol): it is not routinely given after a blood transfusion. It should only be used by a clinician for a specific indication, such as severe symptomatic bradycardia in selected settings.
If there is a suspected transfusion reaction with wheeze, low blood pressure, swelling, or shock:
- Stop the transfusion immediately and keep IV access with normal saline.
- Seek urgent medical help and notify the blood bank.
- Adrenaline (epinephrine), not isoprenaline, is the usual first-line drug for anaphylaxis, alongside oxygen and IV fluids, following local protocol. The transfusion should not be restarted in suspected anaphylaxis or haemolytic reaction. Lifeblood guidance
