Mr Krish malhotra is a 21 years old single male,He is a medical clerk from pune,India .Patient came in with complaint of acne.The acne began when patient was 17 yrs old clustered around forehead and cheeks.With time it become progessively worse .It was assoicated with pus,bleeding and pruritus. The acne was aggrevated by Sunlight,Junk food , sweet food , stress and sweating. It was relieved after washing face with face wash . Patient took consultation with dermatologist and had unrecalled diagnosis . He was given unrecalled antibiotics ,cream which provided relief. .But patient stopped taking medications after 6 months due excessive number of medications after which his acne reappaered.He was also advised to keep the sun exposure to minimum .He has no previous hospitalization and surgical history .Curently patient is taking Vitamin C 2-3times/week. Patient denies any consumption of alcohol,smoking,illicit drugs or over the counter medications.Patient denies having open or closed comedones.Patient denies any allergies to food or drug .His daily diet includes chappathi ,fast food,chocolates and sweet products.Patient reported increased oily skin recently.Patient denies any family history of acne or any other medical conditions. ,Patient uses cold water for bathing and avoid soap due to irritation.He uses Nax anti ache face wash daily 1-2 times/day which provides temporary relief.He uses sunscreen SPF50 daily,denies using any moisturizer,hair oil or hair gel.He reported that he pricks his acne almost daily.Patient denied having feeling of depression,constant need to hide acne, problems with relationships due to acne.Patient denied any discrimination and bullying due to his condition. His review of system is unremarkable. No weight loss ,no fever,no joint pain,No hair fall, no nail changes,no blurry vision.
