low SHBG causes clinical interpretation sex hormone binding globulin

Reduced SHBG means your blood level of sex hormone-binding globulin is low. SHBG is a liver-made protein that carries testosterone and estrogen in the bloodstream.
When SHBG is low, a larger proportion of testosterone may be free (biologically active). It does not automatically mean your total testosterone is high or that you have a hormone disorder. Interpretation depends on your sex, symptoms, lab reference range, total testosterone or estradiol, albumin, and sometimes calculated free testosterone.
Common associations with low SHBG include:
  • Overweight/obesity and insulin resistance or type 2 diabetes
  • Hypothyroidism
  • PCOS, in females
  • Cushing syndrome
  • Use of anabolic steroids or some steroid medicines
  • Aging in some people
This is consistent with MedlinePlus guidance on SHBG testing.
If you share the SHBG value, units, reference range, sex at birth, and related results such as total testosterone, free testosterone, TSH, glucose/HbA1c, I can help explain what the pattern may mean.
