Levine's theory of dental caries explains that caries results from the interaction between an acid-producing process and variables that modify either the tooth or its environment.

Basic mechanism

Following Miller's acidogenic theory, plaque bacteria metabolize fermentable carbohydrates, especially sucrose, producing organic acids. The fall in plaque pH causes enamel mineral to dissolve, initiating demineralization and, if repeated, cavitation.

Levine's two groups of modifying factors

  1. Tooth resistance factors These determine how resistant the tooth is to acid attack:
    • Chemical composition of enamel, including fluoride content
    • Enamel microstructure and degree of mineralization
    • Tooth morphology, especially pits, fissures, and contact areas
    • Tooth position and accessibility to cleaning
    • Post-eruptive maturation of enamel
  2. Environmental cariogenicity factors These determine how cariogenic the surroundings of a tooth are:
    • Diet: frequency, form, and retentiveness of fermentable carbohydrates
    • Dental plaque: amount, composition, bacterial activity, and retention
    • Saliva: flow rate, buffering capacity, clearance of sugars/acids, calcium and phosphate availability
    • Exposure to fluoride and oral hygiene practices

Central idea

Caries occurs when the local oral environment repeatedly produces acid challenges that exceed the resistance and repair capacity of the tooth. It is therefore not caused by bacteria or sugar alone, but by their interaction with susceptible tooth surfaces and protective factors, especially saliva and fluoride.
Levine also described early caries as a dynamic ionic exchange between plaque fluid and enamel. When plaque pH falls, calcium and phosphate leave enamel. When pH recovers, remineralization can occur, particularly in the presence of fluoride.
This framework expands the simple Keyes triad by emphasizing why different teeth and surfaces in the same mouth can have different caries susceptibility. Levine's original review states that enamel characteristics modify tooth resistance, while diet, plaque, and saliva determine environmental cariogenicity in addition to the basic acidogenic process (original abstract).
