I can confirm this appears to be a Raven’s Standard Progressive Matrices (SPM) response record: 5 sets, A-E, with 12 items each, for 60 responses total.
I cannot give a defensible raw score or intelligence interpretation from this image alone because:
- The official SPM answer key is required to score each response, and item A1 is not fully legible.
- A percentile or grade requires the patient’s age, the exact SPM edition/norm table used, and ideally the local normative reference.
- The note “shaky → attention ↓” is clinically important. Reduced attention, tremor, anxiety, delirium, medication effects, fatigue, visual impairment, or poor motor control can lower performance. The result should therefore be reported as potentially invalid or an underestimate unless testing conditions were adequate.

Suggested clinical report wording
Raven’s Standard Progressive Matrices was attempted, with all 60 items apparently answered. Testing behavior was notable for shakiness and reduced attention. Because attention was compromised, the obtained score, if calculated, should be interpreted cautiously and may underestimate the patient’s nonverbal reasoning ability. A repeat assessment under optimized conditions may be appropriate if the result will affect diagnosis, capacity assessment, educational planning, or treatment decisions.
The SPM produces a total raw score that must be converted to an age-appropriate percentile using the correct test manual; it is not appropriate to infer IQ directly from the raw number. Pearson notes that SPM is a 60-item, five-set measure and reports performance as a raw score and percentile relative to a norm group in its SPM overview.
Send the patient’s age, the official scoring key/score sheet for the exact SPM version, and confirmation of A1, and I can total the score and help word a cautious interpretation.

To calculate the score, I still need the official answer key for the exact SPM edition. Please upload a photo of the scoring key or mark correct/incorrect responses. Also provide the patient’s age so the total can be converted to the correct percentile/interpretive grade.
Given the recorded shakiness and reduced attention, document any score as a cautious estimate that may underrepresent nonverbal reasoning ability.

This image is an official SPM answer sheet, not the answer key. It lists items A-01 to E-12 with blank boxes for recording responses, but it does not show which option is correct for each item.
I therefore still cannot calculate the patient’s score from it.
Please upload the page titled “Scoring Key,” “Answer Key,” “Key to SPM,” or a table showing the correct response for A1 through E12. Also send the patient’s chronological age to convert the raw total to an age-normed percentile/grade.
The record of “shaky, attention ↓” should remain in the report as a factor that may make the performance score an underestimate.

I rechecked both images carefully.
- First photo: patient’s selected responses only.
- Second photo: official blank answer sheet only. It contains item labels A-01 to E-12, but no indicated correct options or marks.
So neither image includes the correct answers needed to mark the test. I should not invent a score.
Please upload the page that lists the correct option for each item, for example:

- Total raw score out of 60
- Error pattern across item difficulty
- Interpretation, once you also give the patient’s age and the relevant SPM norm table/edition.

Those are the patient’s chosen option numbers for items A1 through E12, not the item scores. In SPM, each item is scored:
- 1 if the selected option matches the official answer key
- 0 if it does not
- Total = sum of correct items, out of 60
For example, A1 = 6 only scores 1 if the official key says A1 = 6.
The response list alone cannot show how many were correct because it does not contain the keyed correct answers. The prior upload was a blank response sheet, not a scoring key.
If you mark each item as correct or incorrect, for example:
A: 1,1,0,1,...B: 1,0,1,...I can immediately total the five set scores and overall score. To interpret the total as a percentile/grade, I will also need the patient’s exact age and the norm table for your SPM edition.
Do not label this as an IQ score directly. With the observed shakiness and reduced attention, any obtained SPM result should be interpreted cautiously as potentially underestimating current performance.
