Ye diagram kya explain kar raha hai?

Ye Middle Ear Pressure Equalization ka mechanism hai - matlab kaan ke andar ka pressure bahar ke pressure ke saath balance kaise hota hai, aur jab ye balance bigad jaaye toh kya kya problems aati hain.

NORMAL Mechanism (Upar wala part)

Kaam kaise karta hai?

Jab bhi aap aeroplane mein udte ho ya pahad par jaate ho, bahar ki hawa ka pressure change hota hai:
  1. Bahar ka pressure negative ho jaata hai (ya andar ka positive ho jaata hai)
  2. Jab ye pressure difference 15 mmHg cross kar le...
  3. Eustachian Tube (ET) automatically push ho ke khul jaati hai
  4. Kaan ke andar ki hawa bahar nikal jaati hai
  5. Pressure equalize ho jaata hai - donon taraf equal
ET basically ek pressure relief valve hai jo middle ear aur nasopharynx (naak-gala ke junction) ke beech hoti hai. Normally ye band rehti hai aur sirf pressure difference pe khulti hai - jaise nigalne par ya haath se naak band karke phukne par.

ABNORMAL Mechanism - Jab ET Block ho jaaye

Pharyngitis (gale ki soojan), naak ki allergy, sardi, ya koi aur infection ki wajah se ET block ho sakti hai.

Chain of Events:

ET block → Hawa bahar nahi nikal sakti
    ↓
Aeroplane ke ascent mein pressure badhta hai
    ↓
TM (Tympanic Membrane = kaan ka parda) BAHAR ki taraf bulge karne lagta hai
    ↓
Zyada pressure → TM Perforation (kaan ka parda phoot jaata hai)

Aur Zyada Pressure Badhne par - Ossicular Chain Stretch

Agar pressure aur badhe (jaise aeroplane mein tezi se upar jana), toh:
  • Ossicular chain (malleus → incus → stapes - teenon chhoti haddiyan) stretch ho jaati hain
Yahan se 2 alag complications aate hain:

LEFT side (Green) - Oval Window

  • Oval Window bahar ki taraf push ho jaati hai
  • (Relatively less dangerous - hearing thodi affect hoti hai)

RIGHT side (Red) - Round Window - Zyada Dangerous!

Round Window andar ki taraf push hoti hai
    ↓
Round Window RUPTURE ho jaata hai
    ↓
Perilymphatic Fistula ban jaata hai
    (= inner ear fluid - perilymph - bahar leak hone lagta hai)
    ↓
SUDDEN ONSET VERTIGO
    (achanak chakkar aane lagte hain)

Clinically Important Points

  • Pharyngitis/Sinusitis ke waqt flight mein travel karna dangerous ho sakta hai - isi wajah se
  • Valsalva maneuver (naak band kar ke haawa phukna) ET ko forcefully kholta hai - pilots aur divers ye karte hain
  • Perilymphatic Fistula ek serious complication hai - ismein inner ear ka fluid (perilymph) middle ear mein leak ho jaata hai, jisse sudden vertigo aata hai
  • Round window zyada vulnerable hai oval window se, kyunki wo membrane se covered hai (oval window mein stapes ka footplate hota hai)

Ye diagram "During Descent" ka hai - yaani jab aeroplane neeche aa raha ho ya diver upar aa raha ho (opposite of ascent).

During Descent - Kab aur Kyu Hota Hai?

Kab Hota Hai?

Descent = jab aap upar se neeche aate ho:
  • ✈️ Aeroplane landing ke waqt
  • 🤿 Scuba diver jab pani mein neeche jaata hai
  • 🚗 Pahad se neeche utarte waqt (less common)
Diagram mein likha hai - "more common than ascent" - matlab descent mein ear problems zyada hoti hain ascent ke mukable. Kyun? Kyunki ascent mein ET passive tarike se pressure release karti hai, lekin descent mein ET ko actively kholna padta hai (chewing/swallowing se).

→ Middle ear mein blood bhar jaata hai
→ Hemotympanum (kaan ka parda laal/dark dikhta hai - jaise photo mein)
Ye photo mein hemotympanum ka actual clinical appearance dikha rakha hai - parda dark reddish/bluish dikhta hai kyunki peeche blood collect hua hai.
