Weight-Based Dosing Details

  • Infants/toddlers 6 months - 2 years: 0.125 mg/kg once daily
    • If inadequate response or poor tolerability with once-daily dosing, 0.125 mg/kg twice daily can be used (off-label, supported by the EPAAC trial)
  • Children 2-5 years: standard 1.25 mg once daily; up to 2.5 mg twice daily if needed (off-label)

Important Notes

  • Do not exceed the recommended dose - higher doses increase somnolence risk without additional benefit
  • Renal impairment: Levocetirizine is contraindicated in children under 12 years with any degree of renal impairment (the drug is renally cleared)
  • Hemodialysis: Contraindicated
  • No hepatic dose adjustment needed
  • Can be taken with or without food

Clinical Evidence

The 0.125 mg/kg dose was validated in the EPAAC trial - a prospective, double-blind, placebo-controlled study of 510 atopic children aged 12-24 months receiving 0.125 mg/kg twice daily for 18 months, showing a safety profile comparable to placebo.

Note: Always verify doses against local prescribing guidelines. In children under 6 months, safety and efficacy have not been established.
