You're asking whether you can tell if someone has read your iMessage on iPhone.
Here's how it works:

iMessage Read Receipts

"Read" appears under your message when:
  • The recipient has Read Receipts turned ON
  • They opened the conversation and viewed your message
  • Both of you are using iMessage (blue bubbles), not SMS (green bubbles)
You'll see:
  • "Delivered" — message reached their device
  • "Read" + a timestamp — they opened and read it

Why You Might NOT See "Read"

  1. They turned off Read Receipts — the most common reason. They can disable it globally or per-contact (Settings → Messages → Send Read Receipts)
  2. It's a green bubble (SMS) — SMS never shows read receipts
  3. They viewed it from the notification without opening the app — won't trigger "Read"
  4. Do Not Disturb / Focus mode — message delivered but not opened
  5. No internet connection — may still show "Delivered" but not "Read"

How to Check if Read Receipts Are On (Your Side)

Go to Settings → Apps → Messages → Send Read Receipts — toggle on/off.
You can also enable/disable it per contact: open a conversation → tap their name at the top → toggle Send Read Receipts.
So if you only see "Delivered" (or nothing), it doesn't necessarily mean they haven't read it — they may just have receipts turned off.
