Taking rifaximin with or after food is actually recommended - food increases its absorption slightly, though since it is minimally absorbed by design (it acts locally in the gut), the clinical effect on GI flora is largely the same. The main practical reason to take it with meals is to reduce the chance of nausea or GI upset and to make the thrice-daily schedule easier to remember.
If the patient skips a meal, they can still take the dose with a small snack rather than on a completely empty stomach.
