Roko is usually loperamide 2 mg for short-term watery diarrhea.
Adults and children age 12+
Take 2 capsules/tablets (4 mg) initially
Then 1 capsule/tablet (2 mg) after each loose stool
Do not use it for more than 48 hours without medical advice.
Follow the maximum dose printed on your specific pack. Many prescription references permit up to 16 mg/day (8 x 2 mg), but do not exceed the pack limit or take extra doses. The loperamide prescribing information gives 4 mg initially, then 2 mg after each unformed stool.
Do not take Roko without medical advice if you have fever, blood or mucus in stool, severe abdominal pain/swelling, suspected food poisoning or antibiotic-associated diarrhea, or diarrhea lasting over 2 days. Focus on ORS fluids to prevent dehydration.
It is generally not suitable for children under 12 unless a clinician specifically prescribes it. High doses can cause dangerous heart-rhythm problems. The pharmacology text also advises stopping it if acute diarrhea has not improved within 48 hours - Goodman & Gilman’s The Pharmacological Basis of Therapeutics, section “Antimotility and Antisecretory Agents.”
