For a 10-year-old with a simple lower UTI (cystitis), the US oral-suspension label dose is:
  • Nitrofurantoin 5 to 7 mg/kg/day orally, divided into 4 doses
  • That equals 1.25 to 1.75 mg/kg per dose every 6 hours
  • The 25 mg/5 mL liquid concentration equals 0.25 to 0.35 mL/kg per dose, four times daily.
  • Give with food to improve absorption and reduce stomach upset. DailyMed prescribing information
Example: if she weighs 30 kg, the total daily dose is 150 to 210 mg/day, or 37.5 to 52.5 mg every 6 hours. The exact prescribed dose should be rounded to an available liquid or capsule strength by her clinician/pharmacist.
Important: nitrofurantoin is for lower urinary infection only. It should not be used for suspected kidney infection, such as fever, chills, vomiting, flank/back pain, or a very unwell child. Also, the 100 mg twice-daily Macrobid formulation is labeled for ages 12 years and older, so do not substitute it for the pediatric regimen without prescriber instruction.
